Substitute Notice of Data Breach

This substitute notice is provided by Florida Baptist Children’s Homes, Inc. (FBCH) concerning a data breach incident affecting records of a limited number of former residents on the Jacksonville, FL campus.  On October 12, 2018, FBCH learned that two current residential clients in the work program were assigned by a supervisor the task of purging closed client files that met the requirements of the record retention policy. The purged records included information such as name, date of birth, Social Security number and medical information.

FBCH thoroughly investigated the matter, and appropriate personnel repercussions were implemented. To help minimize the risk of future data breaches, FBCH is reviewing appropriate protocol with employees who have authorized access to client files. The protocol ensures that clients are unable to access files pertaining to other current or former clients. Further, if unauthorized people gain access to confidential files, disciplinary action will be taken with those who are responsible.

If you have reason to believe that your information is being misused, you should contact local law enforcement and monitor your credit reports periodically. The Fair Credit Reporting Act requires nationwide consumer reporting companies to provide free copies of credit reports upon request, once every 12 months.
